Adaptive EHR: simplifying medical practice as a complex system

We would like to introduce adaptive EMR/EHR as a way to materialize Patient Centered Management as a solution to sustain the upgrade introduced in Healthcare in the US.

This implies dealing with predictive interfaces transforming operational EMR/EHR into adaptive systems. We would like to discuss with colleagues the possibility of introducing these alternatives considering medicine as the most complex professional field.

The basics of medicine are patient centered. That is why the regulation of the activity focusing on patients is just a back to basics. Patient Centered Management is an object driven solution to structure patient orientation.

Electronic Patient Records are the paradigmatic sign of this approach. Electronic Healthcare records are their natural complement in order to include the aspects that deal with the diseases that are being considered.

Electronic Medical Records, being focused on medical activity, allow putting this patient orientation into action. The EMR are the natural step to start with a structured patient orientation.

The debate I suggest is about alternatives to simplify complexity in medical practice with people who might be interested in this approach.

Innovation is the basis for economic growth.  Essentially, countries, cultures, institutions, and individuals grow only in the fields where they are open to innovations.

The purpose of innovation is to overcome scarcity. That is why only people or cultures that have the will, find the way to overcome scarcity.  This is the cultural context that fosters innovation.

The driver of innovation is human creation. The word creation is used in the sense of recreation. Humans create based on the existing energy.

The maximal strategy of innovations drives towards the solution of a scarcity. Therefore innovative organizations have the perception that they add value to the environment. But this is only true if the minimum strategy is also fulfilled.

The minimum strategy implies inserting the innovations into the social capital of the environment. Social capital is defined by the strength of the bonds that integrate the members of a community.

Commercial innovations need to be only one step ahead of the habits of the environment. If they are more than one step ahead they fail, unless there is an extreme need and they are, implicitly, a “back to basics”.

In this field Procter & Gamble is a notorious benchmark.

Benchmarking Rolls-Royce is necessary for all those products or activities that are considered prestigious by the community and that need to sustain their seeking for perfection to achieve their credibility in the market.

A Rolls-Royce is an extremely prestigious car because it has extreme functionality, quality, added value and reliability.

The integration of these four extreme characteristics makes its positioning as a prestigious car possible.

The purpose of a “Rolls-Royce” is to satisfy the feeling of having achieved perfection. Perfection is considered as something that satisfies 100% the expectancy of the buyer. This means that a “Rolls-Royce” satisfies the needs of the buyer considered as an individual, as part of a group and within her/his social environment.

Every activity or product that requires perfection, in the above described sense, needs to position as a “Rolls-Royce”.  This can be frequently seen in medicine, in politics, in strategy, and in all those fields where the value added is extreme and extreme reliability is expected.

The extreme functionality mentioned above has two aspects. It has to be extremely functional as a device but also extremely functional in its operation. This implies that no one can expect a superior functionality in the market.

The need for extreme quality implies extreme quality assurance and extreme control. Thus reliability and extreme added value are sustained.

Therefore the process of Rolls-Royce building includes an extremely strict concept, its update in the field of scientific possibilities, the management of all the techniques necessary to achieve perfection and last, but not least, the necessary art to make the perfection perceivable by the community.

Rolls-Royce building and positioning is not a question of choice. It is a question of beliefs. But it needs to be said that this does not guarantee success in business.

Dealing with the need to evolve

Parallel realities become a need when individuals cannot deal with their own evolution.

Parallel realities are imaginary situations installed beyond the real world that are built upon fallacious beliefs or needs. Fallacies are functional to humans. Individual fallacies reduce personal energy consumption, and fallacious utopias ensure the perception that something useful will be done.

While utopias function as action drivers, fallacious utopias function as empty “action promises”. Fundamentalism, considered as an objective characteristic an individual needs to survive, requires “living” in a parallel world to avoid self-perception.

To build this parallel world individuals use their projection capacity to deposit on the environment what they need to perceive in order to feel comfortable.

Addictions, the natural response to hopelessness

Addictions are a natural response to an uncertain environment in which a person feels hopeless, impotent and without identity.

When these conditions are given, individuals enter the shelter of addictions.

Addictions have a secondary benefit to help people to deal with hate, anxiety, illusions and guilt. Addictions help to deal with these self-destructive feeling in a paradoxical way. While the individual is installed in the parallel world he feels fulfilled, but, after he returns to the real world, he feels worse than before and the need for addictive stimuli rises.

Addictions are infinite. The more unbalanced an individual is, the “heavier” the addictions he adopts.

Considering the level of self-destruction or destruction four segments can be characterized:

1) Pleasure addictions: providing instants of extreme pleasure.
2) Rational/spiritual addictions: providing sensation of absolute empowerment.
3) Suicidal addictions: providing the power of “dancing” beyond human limits.
4) Terrorist addictions: providing the power of feeling God.

Liberation from guilt: A business as old as humanity

Individuals feel guilty if they cannot overcome what they have inherited.

Guilt corrodes humans’ physical, psychological and spiritual health. Therefore “the liberation of guilt” has been a business that existed since mankind exists. It has been  run from witchdoctors to psychologists.

Psychology opened a new perspective to human health. But it also settled the groundings of extreme individualism. The western hemisphere jumped from a sanctification of parents to blaming them for all the problems individuals have.
